To complete the online application, simply type your answers in the corresponding fields or select your answers from the popup lists (where applicable).  $35.00 application fee is waived when you complete an application online.

Applicant Information
- - - - Expected Entrance Term
The semester in which you are applying for admissions.  We are currently accepting applications for Summer II and Fall 2008.  (Please refer to the Home page of our website for summer session dates and class times.) 

- - - - Initial Program

ORM – Adult and Continuing Education Organizational Management Program applicants
Teacher Certification – post graduate certification program for applicants who have a Bachelor’s Degree
Undergraduate – All other majors

- - - - Demographics

To better assist our office in capturing statistical data for our applicants, please answer all questions applicable to you.

*Social Security Number is required to process your admission application.  Social Security Numbers are kept confidential and will be used to identify you for admission and financial aid purposes.

- - - - Home Address
Address where you receive your mail.  The address you provide will be the mailing address the admissions office will use when sending out admission status and/or acceptance letters.  Please include an apartment number, if applicable. 

- - - - Home Phone Number
Phone number where we can best reach you if we need to contact you regarding your admission to the College.

- - - - Local Address
If you will be residing off campus and your local address differs from your home address, you may provide a local address.  Otherwise, leave blank.

- - - - High School Information and Test Scores
High School and Test Score information must be provided for freshmen students who have
0-15 college credits. 

*Test scores are waived for applicants who are 25 years of age or older. 
If you did not take the ACT or SAT test leave fields blank.

- - - - Transfer College Information

Please provide names of all previously attended colleges, universities, or trade schools.  Credits may not be transferable, but if you received financial aid from any school, an official transcript from the institution is required.

Military Credits will be considered upon receipt of DD-214 Form.

- - - - Adding School Names

If you can not find your school name in the search engine you can add the complete name of the high school or college.  Do not abbreviate.  You will not see your added school when you review your application prior to submission (please do not add it again).  The information is electronically submitted to the admissions office.

- - - - TOEFL

International applicants from non English speaking countries are required to submit TOEFL scores with a minimum score of 400.

Parent Guardian Information
Please provide name, complete address and phone number of your next of kin.
