To better assist our office in
capturing statistical data for our applicants, please answer all
questions applicable to you.
*Social Security Number is required
to process your admission application. Social Security Numbers are
kept confidential and will be used to identify you for admission and
financial aid purposes.

Address where you receive your mail. The address you provide
will be the mailing address the admissions office will use when
sending out admission status and/or acceptance letters. Please
include an apartment number, if applicable.

Phone number where we can best reach you if we need to contact
you regarding your admission to the College.

If you will be residing off campus and your local address
differs from your home address, you may provide a local address.

and Test Scores
High School and Test Score information must be provided for
freshmen students who have
0-15 college credits.
*Test scores are waived for
applicants who are 25 years of age or older.
If you did not take the ACT or SAT test leave fields blank.

Information
Please provide names of all
previously attended colleges, universities, or trade schools.
Credits may not be transferable, but if you received financial aid
from any school, an official transcript from the institution is
required.
Military Credits will be considered
upon receipt of DD-214 Form.

If you can not find your school name
in the search engine you can add the complete name of the high
school or college. Do not abbreviate. You will not see your added
school when you review your application prior to submission (please
do not add it again). The information is electronically submitted
to the admissions office.
- - - - TOEFL
International applicants from non
English speaking countries are required to submit TOEFL scores with
a minimum score of 400.
Parent Guardian Information
